Visa Application Requirements for US citizens

Email Requirements
A valid passport and Bolivian business visa are required for travel to Bolivia.

Passport Requirements:
Original, signed passport valid for 6 months beyond stay in Bolivia, with at least one blank visa page available for Bolivian visa stamp. Amendment pages in the back of the passport are not suitable for Bolivia visas.

Bolivian Visa Application Form:
One visa application form per applicant properly completed and signed. Signature must be original on each application. Improperly completed forms may delay the processing of your business visa.
Washington DC Application: There is an on-line visa application form required in Washington DC. If sending your application to Washington DC, the form must be completed on-line, printed and signed.

Photo Requirements:
One recent 2 x 2 passport type photograph in color, front view and with a plain/white background.

OR - you can upload a passport photo to the Global Service Order Form when you place your order, instead of providing physical passport photos.

Official Invitation from Bolivia:
A letter of invitation from sponsor in Bolivia (either host company or individual) explaining the relationship to the applicant and the purpose of travel. In addition, the letter must indicate the length of stay, place of stay, and contact information.
New York: The original, notarized invitation is required.

Business Letter:
Letter from employer or sponsoring company in the US, on company letterhead, introducing the applicant, indicating the applicant's employment status/position held in the company, and clearly stating the purpose of travel to Bolivia. The business letter must also indicate who will be financially responsible for the applicant and must provide detailed contact information in Bolivia.

To generate the Bolivia Business Cover Letter on-line, fill out this Business Letter Form and print the completed letter on your company letterhead.

Proof Of Departure:
Computer generated flight itinerary from the airline or a travel agency or copy of airline tickets.

Criminal Record:
New York and Miami: Good Conduct Certificate obtained from the Police Department.

Medical Requirements:
International certificate of vaccination against yellow fever.

Financial Requirements:
Evidence of sufficient funds to support stay in destination such as most recent bank statement or a photocopy of applicant's credit card (front side).

Residency Status:
Notarized copy of US Permanent Resident Card or valid visa. US visa holders should have original I-94 or I-20 stapled to their passport.

Global Service Order Form:
This is Travisa's service order form, where you will provide your contact and shipping information, choose the visa processing needed and make your payment. You must print the completed form and send it to Travisa with all other supporting documents.
Bolivia Visa Validity:
Business visas (Specific Purpose) are issued for a single entry, valid for 30 days from the date of issue. Visas can only be extended in Bolivia through the Bolivian Immigration Department (Dirección Nacional o Regional de Migración).
Bolivian Visa Processing Time:

New York: Police Clearance Certificate is required if sending an application to New York.

Minors:
Minors (under 18) are required to provide a notarized letter of consent signed by parent(s) or guardian authorizing the Bolivian Consulate to issue a visa to the minor and specifying who will have responsibility for the minor in Bolivia.
