Yes - A visa is required for travel to Bolivia

Personal appearance required: The consulate requires travelers to apply directly and a personal appearance may be necessary to obtain your visa. We recommend that you contact the consulate to obtain the most current forms and requirements before making a personal appearance at the consulate. Many consulates require an application appointment. Passport Requirements:

Original, signed passport valid for 6 months beyond stay in Bolivia, with at least one blank passport page available for Bolivian visa stamp. Amendment pages in the back of the passport are not suitable for Bolivia visas.

Photo Requirements:

One recent 2 x 2 passport type photograph, in color, front view and with a plain/white background.

Proof of Travel Arrangements:

Computer generated flight itinerary from the airline or a travel agency or copy of airline ticket showing entry and departure from Bolivia.

Hotel Confirmation:

A copy of confirmed hotel and/or cruise reservation(s.) If staying in various hotels, please provide copies of each reservation. A letter from inviting family member or friend is required if they are providing accommodations to the applicant.
Or: a notarized invitation letter from Bolivian friends or relatives indicating the address of the host.

Applicants Under the Age of 18:

Minors (under 18) are required to provide a notarized letter of consent signed by parent(s) or guardian authorizing the Bolivian Consulate to issue a visa to the minor and specifying who will have responsibility for the minor in Bolivia. Minors must also submit parent’s recent bank statement that shows evidence of sufficient funds to support stay in destination.

Medical Requirements:

International certificate of vaccination against yellow fever.

Proof of Sufficient Funds:

New York and Washington DC: A recent bank statement that shows evidence of sufficient funds to support stay in Bolivia.
Miami Evidence of sufficient funds: Three most recent bank statements. Copy of credit card is not accepted.
All other jurisdictions: Evidence of sufficient funds to support stay in Bolivia such as most recent bank statement or a photocopy of applicant's credit card (front side).
