Original, signed passport valid for 6 months beyond stay in Bolivia, with at least one blank passport page available for Bolivian visa stamp. Amendment pages in the back of the passport are not suitable for Bolivia visas.
Bolivian Visa Application Form:
One visa application form per applicant properly completed and signed. Signature must be original on each application.
One recent 2 x 2 passport type photograph, in color, front view and with a plain/white background.

Official Invitation from Bolivia:
A letter of invitation from sponsor in Bolivia (either host company or individual) explaining the relationship to the applicant and the purpose of travel. In addition, the letter must indicate the length of stay, place of stay, and contact information.New York: The original, notarized invitation is required.
Business Cover Letter:
Letter from employer or sponsoring company in the US, on company letterhead, introducing the applicant, indicating the applicant's employment status/position held in the company, and clearly stating the purpose of travel to Bolivia. The business letter must also indicate who will be financially responsible for the applicant and must provide detailed contact information in Bolivia. The Consulate of Bolivia requires an official business letter to support your application. Proof Of Departure:
Computer generated flight itinerary from the airline or a travel agency or copy of airline ticket showing entry and departure from Bolivia.
Houston, New York, Washington DC and Miami: Good Conduct Certificate obtained from the Police Department.
International certificate of vaccination against yellow fever.
Miami, New York and Washington DC: A recent bank statement that shows evidence of sufficient funds to support stay in destination. A photocopy of the applicant's credit card is not accepted.All other jurisdictions: Evidence of sufficient funds to support stay in destination such as most recent bank statement or a photocopy of applicant's credit card (front side).
For multiple entries: a certified copy of Tax Identification Number (NIT) or original electronic certification if the company was established in Bolivia.

Bolivia Visa Validity:
Business visas (Specific Purpose) are issued for a single entry, valid for 30 days from the date of issue. Visas can only be extended in Bolivia through the Bolivian Immigration Department (Dirección Nacional o Regional de Migración).
Bolivian Visa Processing Time:

The US State Department strongly encourages American citizens planning international travel to register with the US Embassy(s) in the countries they plan to visit.
